The NHS Results Framework has five standard areas which aim to improve the quality and outcomes of care and services provided to patients and service users (National Quality Board, 2011). As such, this project plan focuses on Area 2, as was previously mentioned, and is based on improving the quality of people with long-term conditions. Nurses will provide patients with advice on self-care regarding cardiac discharge, identifying how to improve and manage their condition so that they can continue their normal lifestyle. Additionally, patients will be counseled on how to overcome stress and depression, which will help them maintain life activities (DoH, 2013). The National Institute for Health and Care Excellence (NICE) has developed its area of concern for quality improvement in relation to the prevention and treatment of various types of health problems or services. health. Therefore, during this innovation, team members will ensure that patients are safe and not harmed by the change that aims to help them; care is effective, delivered according to best available evidence-based practice and person-centred; ensure that patients are the first concerned when making clinical decisions; avoid unnecessary delays and provide timely care (Health Foundation, 2013). Frances Report (2013) reported on what led to the failure of the care that Mid Staffordshire NHS Foundation Trust provided to patients. He reported that this failure is the result of negligence, lack of good leadership and incompetence of fundamental nursing care on the part of healthcare professionals, which requires a major change in the culture of the NHS across the country.
