    After a thorough assessment of family structure and community dynamics, I would implement a plan that would improve the overall health of the community. Poor community health would be an appropriate nursing diagnosis for this community. According to Ralph and Taylor (2014), the defining characteristics of this diagnosis are: “incidence of risks linked to hospitalization, physiological states, psychological states or health problems encountered by aggregates or a population. No program is available to improve the well-being, prevent, reduce, or eliminate one or more health problems for an aggregate or population” (p. 740). The main objectives of parents would be: to instill in their children good eating habits, to stop smoking and to encourage physical activity and well-being. Another goal for families would be for the parent to express a desire to know what resources are available, to identify local resources and how to access them. Given that the majority of the population in this community is obese and has a history of diabetes and heart disease, I would encourage parents to educate themselves by learning more about the risk factors and consequences associated with obesity, heart disease and diabetes. Since many families in the community receive some sort of government assistance, I encourage families to regularly visit wellness clinics and get checked out to assess their current health. I would also reach out to local food pantries to offer healthier items so these families can incorporate healthy eating into their lives. I would contact community leaders to organize free workshops at churches or other community locations that these families could attend and learn how to maintain good health and prevent complications from obesity and diabetes, etc. I would also encourage local educators and community leaders to provide insight
