She describes that she worries a lot about a lot of things. She has difficulties in her social situation, sometimes has intrusive thoughts. She likes things to be in order. She likes routine. Describes having superstitious compulsive behavior, describes panic attacks, frequent headaches, change in appetite, and reduced energy. She has difficulty going to places with lots of people and social situations are mostly problems when she needs to speak in front of others, but she can go to large groups. She reports hearing voices and occasionally seeing shadows. She reports hearing voices throughout the day, both during the day, while falling asleep and waking up. She reports that the voice told her to harm herself and others. No specific person is identified. She acted accordingly by scratching herself and she never acted on it by hurting others or attempting suicide. Hears voices that are command type and has more than one voice and does not recognize them. Does not relive events that happened in the past, sometimes feels paranoid and has visions of things that others do not see. No change in appetite, comfortable with his grades. The grades are good, but she feels that hearing things is now interfering with her school. Have never used weed, alcohol, cigarettes, drugs, heroin, cocaine or any other drug. Reports that she was never sexually assaulted. She was bullied when she was in third grade. She is not in a relationship. Not sexually active. She identifies as pansexual, "I like everyone" and is active in LGBTQ clubs at school and would like to become more involved in clubs outside of school. Has no confusion about sexuality.
