In addition to what is offered educationally, McGregor PACE scores very high in the area of care support, on the tool patient- and family-centered organizational self-assessment (self-assessment tool, 2013). Family members are an integral part of the McGregor PACE program. Without them, most participants would be placed in a long-term care facility. They are undeniably active members of the team. Family members have full access to their loved ones while on site. If the participant is hospitalized or temporarily placed in a long-term care facility, McGregor PACE is still responsible for monitoring the participant's care (McGregor PACE website, nd). Family is also welcome in these settings, but is subject to visitation guidelines specific to each facility. Families are encouraged to attend the morning meeting, which includes all members of the nursing/medical team. Sentinel or adverse events are immediately shared with the participant/family. Multidisciplinary support is available 24 hours a day, 365 days a year. A doctor is on call 24 hours a day. If the participant experiences a medical emergency or requires resuscitation efforts, family may be present. This does not happen often, as the family does not stay with the participant during site hours. Medications are sent home monthly and as needed. Some medications are administered in the clinical area, but most are administered by family members while the participant is at home. All relevant information is provided in writing to the participant/family whenever medications are sent home or when changes have been made to medications.
